It is now possible to refer yourself directly to a physiotherapist at South Hams Hospital. They are able to help with a variety of musculoskeletal disorders such as back, hip, knee, neck and ankle problems.
If you would like to refer yourself to them please complete the online self-referral form here . If you are not online, you can collect a self-referral form from our reception. Once completed, please hand this back to the reception team and we will send this to the physiotherapy department for you.

Physiotherapy Self- Referral Form
You will be contacted by phone or letter as soon as possible and will be offered an appointment at the physiotherapy department at South Hams Hospital. There can be a delay in being offered an appointment during busy times. Your first appointment will last about 3/4 hour. If you need to be seen again, the physiotherapist will suggest you attend a follow up appointment which will usually lasts 30 minutes.
